I called Medicare today and they told me that we continue using place of service code 11 for office visits if we are not using a TeleHealth virtual software. So basically any office visits, nursing home visits etc. would be the same place of service codes we normally use just append the 95 modifier.

Dr. Kotlar, What types of exercises would you recommend for posture correction that could be billed as a 97112? I have been billing 97110 as well as 97112 for some time now and want to be sure that I am using them properly. Typically when I bill a 97112 for posture correction I have the patient lying supine while on posture "wedges" (lumbar, thoracic, and cervical) that hold the spine in a fixed neutral position. I then have the patients contract and relax their core muscles to get their muscles used to being in that neutral posture position. Would you suggest adding some other variable to that to make it more complient. I have excellent results combining that form of 97112 with different 97110 procedures.

For 2018 MIPS what if you see 200 Medicare patients but do not bill $90,000. Are you still excluded?

If you perform 6 min. of 97032 and 6 min. of 97110, then choose either code to bill - you cannot bill both because the total time is 12 min. Your documentation must show that you did both, however you can only bill for one code.

Regarding timed codes and the -52 modifier? If I do 6 minutes of 97032 and 6 minutes of 97110, or 7 minutes of 97110, then just reporting for one of them doesn't accurately reflect the service performed. How would we be compliant just billing for one unit of 97110? Where is the 97032 reflected? In the notes but not billed for?
